Telephone Services Request Form to Change

Enterprise Information Technology Services (EITS) is coordinating with Finance & Administration (F&A) to transition one of two forms for requesting telephone-related services from a website hosted by F&A to a site hosted by EITS.
Currently, users may access those forms via a fanda.uga.edu or a busfin.uga.edu URL.
Effective July 1, 2017, the form for telephone incidents/repair requests will move to a website hosted by EITS. Anyone with a MyID can access this form.
The form to request telephone incidents/repairs will be available at http://telephoneservices.uga.edu. Click on Incident/Repair on the sidebar for instructions on how to submit the proper request.
This change on July 1 will not impact the form to request a move, add or change to telephone lines, which will remain at busfin.uga.edu. Plans are to transition this form to EITS at a later date.

If you have previously bookmarked the form for requesting telephone incidents/repairs,
please be advised of the new URL. When hosted by EITS, the web form will ask you the
same questions as the current form, but will have a new look, starting on July 1.
